1. The complainant has to be of minimum 18 years old.
2. The complainant has to enter valid e-mail id and mobile no.
3. During lodging the complaint, the complainant has to validate the OTPs sent to both e-mail id and mobile separately.
4. For the complaints related to
(i) Corporate/Regional offices of NALCO, select Corporate Office, Bhubaneswar, Vigilance from the drop-down list. (ii) Smelter & Power complex, select Smelter & Power Complex, Angul, Vigilance from the drop-down list. (iii) Mines & Refinery complex, select Mines & Refinery Complex, Damanjodi, Vigilance from the drop-down list.
5. The signed and scanned documents in PDF or image formats, are to be uploaded and each should be of max. 3 MB size. 6. Please upload documents for 'ID Proof', e.g. Aadhaar Card/ Voter Id/ Driving Licence / PAN / Passport. 7. Please upload documents for 'Address Proof', e.g. Electricity Bill/ Telephone Bill/ Ration Card / Aadhaar Card / Passport. 8. For checking the complaint status, the complainant has to enter the complaint reference no. and to validate the OTP sent to mobile.
